Incident Investigation Results – Piping Failure in HCGO Circuit

In April 2016, a fire broke out in the Lyondell Houston Refinery Delayed Coker Unit. The fire was effectively contained and no injuries resulted from the release. The incident investigation findings will be shared in this presentation. The core lessons learned are focused around proper inspection practices and positive material identification (PMI) in alloy circuits. The presentation will also review how the site reacted to the release in a safe manner to bring the unit into safe park mode and limit the release of hydrocarbons through proper isolation.
