Coker Valve Interlocks

    Nick Willis, a process engineer with ExxonMobil, was reviewing the Chemical Safety Board website and found that they have various videos posted on safety incidents that have occurred in the PetroChemical Industry over the years.  Many good lessons learned are available on this open website, that you may want to share with your coworkers at Safety meetings or as a prelude to your regular weekly, monthly or quarterly team get-togethers.
     
    Here is a link to an investigation of a specific incident in which a valve interlock system was bypassed. Classic normalization of deviance.  The video and report highlight the danger of continually bypassing an interlock as part of normal operation.  This is very applicable to Delayed Coker Valve Interlock Systems:
